When my office contacts you, you will receive an email link to the TherapyPortal (HIPAA compliant)to download the intake paperwork. You can then fill it out and sign the necessary documents and permissions via e-sign; it is time-sensitive.

You can also print the paperwork to fill it out; however, you will still be asked to e-sign documents, payment authorizations, consents, and HIPAA forms.
