Jennifer Baker LPC RPT ACS is an out-of-network provider and does not accept insurance, so payment is due at the time the service is rendered. 

 

She may bill your insurance for you electronically if you choose,  as a convenience, and they send a check to you. 

Please check with your insurance to see if you are covered and what the reimbursement would be, if any.

 

 If you have health insurance, and the services you are seeking are covered by your health care plan, you may be able to get the items or services described in this notice from providers who are in-network with your health plan.


Questions to ask your insurance company:
1. Does my policy include outpatient mental/behavioral health coverage?
2. Does my plan cover out-of-network LPC's?
3. What % of the fee do they pay for OON LPC's?
4. Do they cover the codes being used?
5. What is my mental health/behavioral health deductible and how much has been met so far?

By booking an appointment: The client, Policy holder (s), and Legal Guardian (s) are legally bound & responsible for managing your account and payment of professional services rendered regardless of your coverage.

Your insurance company may be billed electronically, or you can request a receipt,  and the following codes may be  used, but not limited to:
90791 (intake 60 minutes) $190
90834 (therapy session 45+ minutes) $160
90847 (family with patient present) $160
90846 (family session without patient present) $160


**To Cancel Your Appointment 24 hours in Advance: Please Log in, then click on your Login name (top right), you will see your appointments then click the ^ (drop down symbol next to your appointment type) and click on the trash can icon. 


Please be mindful to book your session near your location by clicking on the top right "All Staff" button while viewing the booking calendar. 

All cancellations within 24 hours of your appointment are charged the full fee.

Good Faith Estimate for Health Care Items and Services:
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises
