This is the intake appt AFTER you speak to Jennifer Baker LPC RPT ACS and send the relevant PHI in a manner of your choosing to receive the Therapyportal HIPAA compliant intake paperwork.

Cancellation Policy

There is a 24-hour required cancellation and/or No Show policy of being charged the FULL FEE for your appointment.
